Persuant to A.R.S. 15-872, A.A.C. 702 and 706, a student must be current with the following immunizations to attend Kindergarten through 12th Grade:
  • Diphthera, Tetanus, Pertussis (DTaP or DT or DTP)
  • Polio (IPV)
  • Measles, Mumps, Rubella (MMR)
  • Hepatitis B (Hep B)
  • Tetanus and Diptheria Booster or Tetanus, Diptheria, Pertussis Booster (Tdap) (Required for students 11 years and older)
  • Varicella (Chicken Pox)
  • Meningococcal (Meningitis) (for students age 11 and older)


Have a current exemption form in the approved Department of Health format on file at school.
