Records Requests

There are several types of records which may be requested through the Records Department:
 
 
 
 
  

Public Records Requests

Including:
  • HR/Personnel records (you are a current or former employee of the District)
  • Employment/ Work Experience verifications
  • District Financial records
  • Commercial requests for student information* 

*Due to strict compliance with the Family Education Rights and Privacy Act law (FERPA), please be advised that Dysart Unified School District does not grant requests for student directory information for commercial purposes.  Please visit the Student Directory webpage for more information.

Student Records Requests

Requests for student records include:
  • Special Education Records
  • High School Transcripts (Grades 9-12)
  • Student Education Records
  • Student Health Records
  • Education/Graduation Verification  


Student Directory Information

These requests include Personally Identifiable Information of students. Dysart Unified School District follows strict guidelines regarding who may receive this information.  Please refer to the Student Directory Information webpage for details.

Subpoenas

Subpoenas may be requested for a variety of reasons, including:
 
  • Student Records
  • Workers Compensation Claims
  • Family Court Issues
  • Notices of Claims
