HIGH SCHOOL TRANSCRIPT REQUEST PROCEDURES

Individuals requesting transcripts must complete a “Request for Transcripts” form and provide a legal
form of identification (e.g., driver’s license). The form may be completed on-line, printed and submitted
in-person, via fax, e-mail, or by US mail to the appropriate site listed below. Please ensure you include a
copy of a picture identification card (driver’s license, etc.) (Please set your copier on its lightest setting for
legibility of the identification card.)

Former students, age 18 years of age or older, must submit a written request before a transcript may be
released. If a student is under the age of 18, a parent or legal guardian must sign on behalf of the
student.

OFFICIAL TRANSCRIPTS WILL BE SENT DIRECTLY TO COLLEGE OR EMPLOYER

ATTENDANCE YEARS FROM 2003-04 TO PRESENT

If student graduated or attended after school year 2002-2003, requests should be sent directly to the
current or last school of attendance (see listing below).

ATTENDANCE YEARS PRIOR TO 2003-04

Requests for transcripts for graduation or attendance dates prior to 2003-2004 must be sent to Archives
at the District office.
